Dementia Pathways. For Primary Care in ireland

OTs are members of the primary care team and work closely with their multidisciplinary team colleagues. They see people in their own homes or in health centres. OTs usually get referrals from GPs, PHNs and other health professionals, in some areas self-referrals are also accepted.  

Occupational Therapists provide a central role in the care of people with dementia in the community. They have a role in the following aspects of care for people with dementia:

  • ADL assessments
  • Seating assessments and provision of specialist wheelchairs
  • Assessments of the home environment
  • Provision of  adaptive equipment to promote function
  • Advice on environmental modifications to promote function
  • Advice on use of memory strategies
  • Advice on use of assistive technology
  • Promote engagement in meaningful activities
  • Signposting to support services
  • Support and advice for families
