Dementia Pathways. For Primary Care in ireland

Public Health Nurses (PHNs) and Community Registered General Nurses (RGNs) provide a range of nursing services in the community, based upon standards of best nursing practice. PHNs are often the first point of contact for people to access a range of HSE services. They are based at Local Health Centres and provide care to clients and families in specific geographical areas. Services may be provided in people's homes, health centres, schools, day care centres and other community settings. PHNs/RGNs are members of Primary Care Teams and have access to a wider network of Health Care Professionals for referrals and support.  Contact details for your local services can be found on the Health Service Executive website
