Draft Model of Care for Dementia in Ireland



Dementia is a life-changing condition that requires responses to support people across the illness trajectory, from the point of concern about cognition to receiving a diagnosis, through to early intervention and the provision of appropriate care in the right setting. The projected increase in the number of people who will develop dementia into the future will be a challenge for both diagnostic and post-diagnostic services and there is a requirement for care pathways that are carefully considered, costed and implemented.


This Dementia Model of Care outlines care pathways within the Irish health and social care system for people living with dementia; from identification of symptoms/signs, through to assessment, diagnosis, disclosure, care planning and post-diagnostic support.

The model sets out a range of targets and practice recommendations to advance the treatment, care and support for those living with dementia in Ireland.
